Patient came to opd with the c/o diminision of vision since 2years (near vision)

Patient was apparently asymptomatic 2years back the he developed diminision of vision which is near vision

Oriented to time ,place , person

No pallor, icterus , cyanosis, clubbing, koilonychia , lymphadenopathy,edema

PR-72 Bpm

RR -16cpm

Systemic examination

CVS - S1S2+,NO Murmurs

RS -BAE+,NVBS HEARD

P/A-SOFT ,NON TENDER

CNS-NAD

Opthalmology referral done
